IDs may become mutilated due to water damage, heat exposure, fading of print, cracking, bending, or natural wear and tear. When Is an Affidavit of Mutilated ID Required?
This affidavit is commonly required for the replacement of:

- School or company IDs
- Bank-related IDs or access cards
- Membership cards
