BASIC PASSPORT REQUIREMENTS
(BRING ORIGINAL AND PHOTOCOPY)

1. NEW APPLICATION – ADULT

  • Malinaw na kopya ng PSA Birth Certificate (dapat tama ang spelling at detalye ng aplikante).
  • Malinaw na kopya ng PSA Marriage Certificate kung kasal.
  • Local Civil Registrar Copy ng Birth Certificate kung malabo o hindi mabasa ang PSA copy.
  • Isang (1) Valid ID.

MGA TINATANGGAP NA VALID ID (UPDATED 2025)

  • SSS Card (Social Security System) – Government-issued ID na nagpapatunay ng pagiging miyembro ng SSS.
  • GSIS Card (Government Service Insurance System) – Para sa mga empleyado ng gobyerno bilang patunay ng aktibong miyembro.
  • UMID Card (Unified Multi-Purpose ID) – Pinagsamang ID ng SSS, GSIS, PhilHealth, at Pag-IBIG.
  • Driver’s License (LTO) – Maaaring tanggapin ang student permit kung ito ay nasa card format.
  • BLTO Driver’s License – Tumatanggap lamang ng mga inisyu mula 2022 pataas.
  • PRC ID – Para sa mga lisensyadong propesyonal.
  • OWWA E-Card – Para sa mga aktibong miyembro o OFWs.
  • COMELEC Voter’s ID o Voter’s Certificate – Dapat galing sa COMELEC Main Office, Intramuros, Manila.
  • PNP Firearms Permit – Inisyu ng Philippine National Police bilang valid ID.
  • Senior Citizen ID – Para sa mga aplikanteng may edad 60 pataas.
  • Airman License – Dapat inisyu mula Agosto 2016 pataas.
  • Philippine Postal ID – Valid ID na inisyu ng PhilPost (Philippine Postal Corporation).
  • Seafarer’s Record Book (SRB) o Seafarer’s Identity Document (SID) – Inisyu ng MARINA mula Pebrero 2020 pataas.
  • Valid o Huling Passport – Maaaring gamitin kung para sa passport renewal.
  • School ID –
    • Para sa mga minor: kung walang school ID, maaaring magsumite ng Certificate of Enrollment na may larawan ng bata at dry seal ng paaralan.
    • Para sa adult applicants: School ID at Certificate of Registration (COR) ang kinakailangan.

  • PSA Birth Certificate ng bata
  • Special Power of Attorney mula sa ina kung hindi siya ang kasama
  • Kung nasa ibang bansa ang ina – SPA na may tatak ng Philippine Embassy
  • Valid ID ng ina o authorized representative
  • ID ng minor (7 years old and above)

Tandaan: Para sa batang magtatravel na hindi magulang ang kasama, makipag-ugnayan sa DFA office para sa karagdagang requirements.

  • Police Report (kung applicable)

Paalala: Maaaring magbago o madagdagan ang mga kinakailangang dokumento depende sa pagsusuri ng DFA sa araw ng inyong appointment.
