Pumunta sa https://onlineappform.passport.gov.ph/  - Ang online appform na ito ay kahalintulad ng ONLINE PASSPORT APPOINTMENT ng DFA ngunit ito ay isa sa solution sa mga nagkaroon ng maling datos para sa online appointment confirmation. Mas tinatawag ko itong correction form. Lahat ng ilalagay sa form na ito ay dapat ay tama at walang mali. 

CORRECTION FORM OR ONLINE APP FORM

Step 1: Pumumunta sa https://onlineappform.passport.gov.ph/ 



Step 2: Piliin kung ano ang inyong application type. NEW, RENEW or LOST



Step 3: Lagyan ng tamang impormasyon sa parte ng iyong personal information. Siguraduhin na tama ang spelling ng mga ito.


Step 4: Lagyan ng tamang impormasyon ang tungkol sa iyong magulang. Siguraduhin tama ang spelling ng mga ito.



Step 5: Lagyan ng impormasyon ang iyong contact details. 



Step 6: Summary: Dito ay tingnan mabuti ang mga impormasyon kung tama. Walang hinahabol na oras dito kung kaya't matitingnan mabuti kung tama o may mali pa na nakikita na impormasyon. Kung wala na makitang mali ay pindutin ang SUBMIT button para sa final. 

Step 7: May files na ma-do-download at ito ang isa sa dadalhin pagpunta mo ng DFA OFFICE sa araw at oras ng iyong schedule. Iprint ito at isama sa orihinal na APPOINTMENT PAPER.
