HOW TO REQUEST SSS E-1, E-4, RS-1, NW-1 & OW-1 RECORD?

PAANO KUMUHA ONLINE NG RECORD  SA MGA SUMUSUNOD:

E-1 (PERSONAL RECORD)
E-4 (MEMBER'S DATA AMENDMENT)
RS-1 (SELF-EMPLOYED DATA RECORD)
NW-1 (NON WORKING SPOUSE RECORD)
 OW-1 (FLEXI-FUND FOR OFW)
FORM/RECORD SA SSS ONLINE?

Sa prosesong ito kailangan ay mayroon kanang online account sa SSS. Kung wala pa kailangan mong gumawa muna ng account online.


Sundan ang sumusunod na gagawin para sa pag-request ng E-1, E-4, RS-1, NW-1 & OW-1 Record:


STEP 1:  Login to MY.SSS Online account.


STEP 2: Pagkabukas ng inyong dashboard, makikita ng E-SERVICES at hanapin ang REQUEST RECORD at i-click ito.


STEP 3: Sa REQUEST RECORDS piliin kung ano ang inyong irerequest na form. Lagyan ito ng check at pindutin ang SUBMIT.


STEP 4: (Example) Nilagyan ko ng check ang E-1 Personal Record. 


STEP 5: Buksan ang inyong email address na kung ano ang nakalagay sa record sa inyong SSS Account at tingnan ang email na manggagaling sa SSS. Makikita ang Transaction Reference Number na ibig sabihin ay naka-process na ito. Maghintay na susunod na Email ng SSS.


STEP 6: Bagong email ang matatangap na galing sa SSS. Ito ang email na kung ano record ang inyong nirequest.
